The Department of Foreign Affairs now require you set an appointment before you can apply for a passport, which can take 2 months at the least. Don't waste your appointment by being turned down on the first step simply because of your failure to bring the necessary documents. Before leaving your house, make sure you have the following.
1. The printed application form that was sent in your email when you signed up for the appointment. A link was sent to your email, and clicking on it will redirect you to a PDF file containing 2 pages, one with the date and time of your appointment, and the other being your application form. 2. Bring only valid IDs. And when the DFA says valid, it means identification cards with your picture, signature, date acquired, date of expiration and serial codes like the following - voter's ID, SSS, driver's license, PRC, BIR, Senior Citizen's ID and school ID for students. Voter's ID should be enough if you have the next requirement. 3. Birth Certificate issued by the NSO in SecPa (security paper). SecPa is the yellowish paper your birth information are written on, coupled with a mark from NSO. If BC SecPa is unreadable, you have to get a Certified True Copy (CTC) of BC issued by the Local Civil Registrar with dry seal. 4. All documents to be submitted must have at least one photocopy. 5. For renewals, you have to present your old passport as well as a copy of the first and last page of your passport. 6. No need to have your passport IDs taken before going to the DFA. Passport picture will be taken inside the DFA, during your 3rd step in the process, which is encoding your biometrics information. When I went there for my passport renewal, I noticed that a lot of applicants were turned down, or were asked to come back until they have already completed the required documents. I did not experience such a thing. I only submitted my duly accomplished application/appointment form, BC in security paper (SecPa) with photocopies and presented my voter's ID and the process went really smooth.
